Nobody writes this page, so women planning a three or four day motorbike trip through mountain villages end up guessing about the thing that will actually shape their days: where the toilets are, what they are like, what to do with what you carry out, and whether cramps and a bike seat are a combination anyone survives.
They are, and plenty of people do this trip on their period every week without it becoming the story of their holiday. But the logistics are different from a city trip and it helps enormously to know what you are walking into.
So here is the practical version, without euphemism and without a health lecture. There is no medical advice here, because that is not our job and you did not come here for it. What follows is what this specific trip is like and how to plan for it

Because the standard advice is written for cities. Carry supplies, use a public toilet, buy what you need at a pharmacy. All reasonable, and all of it assumes infrastructure that does not exist between Quan Ba and Dong Van.
On this trip you are on a mountain road for most of the day, the towns are an hour or two apart, and the stops in between are viewpoints with no facilities whatsoever. That changes what you pack, where you stop and what you do with things afterwards.

The thing everyone wants to know and nobody publishes.
In towns, meaning Ha Giang City, Dong Van, Meo Vac, Yen Minh, Cao Bang City, Trung Khanh: proper toilets in restaurants, cafes, hotels and guesthouses. Usually fine. Sometimes squat, sometimes seated, generally functional.
At the main sites, meaning the waterfall, the cave, the bigger viewpoints and the boat station: facilities exist, quality varies enormously, and there is often a small fee. Take your own paper.
At homestays: varies more than anywhere else. Some have modern bathrooms with hot water and a door that locks. Some have a basic room outside the main house. Ask at the booking stage rather than on arrival.
Between all of those: nothing. Roadside stops are viewpoints and lay bys with no facilities, and the honest answer for a gap between towns is behind the bike, off the road, with somebody standing where they can see you coming.
What to expect even in a functioning toilet:
None of this is dramatic. It is just worth knowing before rather than discovering at an inconvenient moment.

The single most useful adjustment, and it costs nothing.
Loop days run between towns with long stretches in between. If you know you will want a proper toilet mid morning and mid afternoon, the day can be shaped around that, because the towns are where lunch happens anyway.
How to do it without a conversation you would rather not have: ask your guide at the start of the day where the town stops are and roughly when. That is a normal planning question anybody might ask, it gives you the whole map of your day, and nobody thinks anything of it.
If you are comfortable being direct, telling a guide you need more frequent stops means they will build them in, and a good guide will do it without comment or fuss. More on that below.

The part generic travel advice gets wrong.
Vietnamese plumbing in rural areas cannot handle paper, and it certainly cannot handle anything else. Flushing products causes real problems for a homestay family’s septic system, and that is a genuinely unkind thing to do to your host.
The solution is simple and it works:
If a bin is provided in a bathroom, that is what it is for and using it is completely normal here. If there is no bin, carry it out.

Useful to know, and it varies enormously by product.
Pads are widely available, including in small village shops and any pharmacy or minimart. Brands are different from home but the products are familiar. This is the easy one.
Panty liners are similarly easy to find.
Tampons are much harder. Available in Hanoi at large supermarkets, international chain stores and some pharmacies. In Ha Giang City you may find them and may not. In the villages, essentially never. If you use tampons, buy in Hanoi before you travel north or bring them from home.
Menstrual cups and discs are not something to count on finding anywhere, including Hanoi.
Painkillers are available at pharmacies in the towns, though names and formulations differ. Bring what you know works.
The rule: stock up in Hanoi, top up in Ha Giang City or Cao Bang City if needed, and do not plan to buy anything in a village.

Worth thinking about specifically, because the answer is different on a mountain road than at home.
A cup or disc has a real advantage here: nothing to carry out, nothing to dispose of, and long wear time that covers a full riding day between towns. For people who already use one, this trip is a good argument for it.
The complication is emptying and rinsing. That needs clean hands and, ideally, water. In a town toilet or a homestay bathroom, fine. At a roadside stop, harder. Hand sanitiser and a small water bottle solve most of it, and planning the empty around town stops solves the rest.
This is not the trip to try one for the first time. Learning how a cup works while sharing a homestay bathroom is a bad week. If you want to switch, do it a couple of cycles before you travel.
Period underwear works well here as a backup layer under riding clothes, and it removes the anxiety about a long stretch between toilets. The complication is drying: humidity in summer and cold in winter both mean slow drying in a homestay, so bring enough pairs that you are not depending on one drying overnight.
Whatever you use at home is usually the right answer. A multi day trip in a country you do not know is not the moment for an experiment.

The practical realities, from people who have done it.
Sitting on a bike is not inherently worse than sitting anywhere else, and some people find the movement and the distraction genuinely helps. Others find the vibration and the position makes it worse. You will know which you are within an hour.
Pillion is easier than riding yourself, because you can shift position, lean, stretch and stop paying attention to the road. On an easy rider trip you are a passenger with somebody else doing the work, which on a day with cramps is worth a great deal.
A jeep is easier again. You can sit back, change position properly, keep warm, hold a hot drink, and stop whenever you want without anyone needing a reason. For a day when you feel rough, this is the version of the trip that lets you still enjoy it rather than endure it.
The one genuine safety point on this page: severe pain is distracting, and riding a mountain road while distracted is a real risk, the same as riding tired or riding upset. If the pain is bad enough to occupy your attention, do not be the one steering that day. Ride pillion or take the vehicle. That is not a comment about anybody’s toughness, it is the same advice we would give a rider distracted by anything else.
Do not take anything that makes you drowsy and then ride. Obvious, and worth saying.
Say you want a slower day. Guides adjust the pace constantly for all sorts of reasons and nobody needs to know why.

Worth its own note because it is specific to this trip.
The mountains are cold at dawn in every month of the year, and genuinely cold between December and February. A lot of people find cold makes cramps noticeably worse, and sitting on a bike in moving air at seven in the morning is the coldest you will be all day.

There are swimming stops on both routes: the waterfall at Du Gia on the four day Ha Giang loop, and the Quay Son river in Cao Bang.
Plenty of people swim on their period with a cup or a tampon and think nothing of it. With a pad it is not practical.
Plenty of people skip it, and that is an equally fine choice on a trip with a viewpoint every ten minutes.
The water is cold, which some people find helps and others find makes things worse. You know which you are.
There is nowhere to change privately at most swimming spots. A sarong or a large scarf handles it, and people manage this constantly.

The honest picture, because it varies enormously.
Dormitories are standard on most tour rates, meaning a shared room with mattresses or beds, and shared bathrooms.
Private rooms are an upgrade and worth requesting, for privacy generally and for this specifically. On our tours the standard rate covers dormitory accommodation and a private room is available as an upgrade, so ask when you book rather than on the day.
Bathrooms range from modern to basic. Some are inside the house, some are a separate building across a yard, some have hot water and some do not. Locks are not universal.
Ask specific questions when booking: private or shared bathroom, is it inside, does it lock, is there hot water. Operators know which places have what, and this is a completely ordinary thing to ask.
Early mornings are the quiet time. If privacy matters to you, getting up fifteen minutes before everyone else is the simplest solution to a shared bathroom.

Homestays are people’s homes, which shapes the answer.
Ask before washing anything, and ask where. Most hosts are entirely relaxed about it, and asking takes ten seconds.
A small amount of travel wash and a dry bag lets you handle things yourself without involving anybody.
Drying is slow. Humidity in summer and cold in winter both mean an overnight wash may not be dry by morning. Plan for that rather than depending on it.
Do not put anything in a homestay’s laundry without asking directly.
The awkward bit, so let us be practical about it.
You owe nobody an explanation. “I need to stop” is a complete sentence and any decent guide responds to it without asking why. Use it as often as you need to.
Most guides on these routes are men, which is simply the reality of the industry here, and it is why some women would rather not explain. That is entirely reasonable.
If you are comfortable being direct, a guide who knows will plan the day around town stops rather than scenic ones, which genuinely improves your day. Guides here have taken thousands of guests through these mountains and nothing about this is new to them.
A middle option that works well: tell them at the start of the trip that you will need more frequent stops, without any explanation. That is all the information anyone needs to plan a good day.
If a guide ever makes you feel awkward about it, that is a problem with the guide and worth telling the operator, because they will want to know.

Worth asking about, and worth being honest that it is not always available.
Most motorbike operators in Ha Giang have few or no female guides, which is a real gap in this industry rather than a shrug. If having a woman on the trip matters to you, ask at the enquiry stage rather than assuming, and ask before you book rather than after.
Some alternatives that help: booking a private trip rather than a shared group, travelling with a friend, or taking a jeep where the daily rhythm is more flexible and less exposed.

Some people plan around it and some do not bother, and both are fine.
If your cycle is predictable and you have flexibility on dates, planning around it is the simplest solution and costs you nothing.
Travel disrupts cycles. Long flights, time zones, altitude, unfamiliar food and disrupted sleep all affect timing, so plan for the possibility that it arrives early, late or unexpectedly even if you were confident about the dates. Pack accordingly regardless of when you think it is due.
Delaying a period with contraception is something a lot of travellers do. It is a conversation with your own doctor, not with us or with a blog, and it needs to happen well before you travel rather than the week you fly. We are not going to say anything more specific than that.
If you have a condition that makes this genuinely difficult, talk to your doctor about the trip specifically. A multi day mountain motorbike route is a particular kind of demand and your doctor can advise where we cannot.

A jeep is the most comfortable option on a difficult day and it is not a consolation prize. You can sit back, change position, stay warm, keep a bag next to you, stop whenever you want without needing a reason, and reach a town toilet on your own timing rather than the group’s. Easy rider means you are a passenger with an experienced local rider doing the work, which is genuinely easier than riding yourself on a rough day. You can shift position, stretch and stop concentrating, and asking to stop is normal on these trips.
Self drive if you ride at home and hold the correct licence and International Driving Permit issued under the 1968 Vienna Convention. The 1949 style permit is not recognised in Vietnam, and the paperwork guide covers it. The only thing to add here is the one already said: if pain is occupying your attention, do not be the one steering.
Mixed and flexible works. Riding some days and taking the vehicle on others is an entirely normal arrangement and nobody needs to justify which they are doing on any given morning.
Book once your dates are set. Most guests book one to three months ahead, and jeep availability is tighter than motorbikes.
